MaxCare© Program
CARE Fertility offers an extended treatment program for patients who understand that successful treatment may often involve more than one cycle of treatment. Our program, MaxCare©, includes up to 12 months of continued treatment (until a successful pregnancy is achieved) at a discounted rate. In order to participate in MaxCare©, patients must first meet our acceptance guidelines for medical clearance outlined below:
Female Requirements:
- BMI – Current body mass index must be below 40.
- Bloodwork - (Must be completed within the past 3 months)
- TSH
- Day 2, 3 or 4 FSH
- Prolactin
- Testosterone
- AMH
- Day 2, 3 or 4 LH
- Day 2, 3 or 4 Estradiol
- Day 2, 3 or 4 Sonogram for antral follicle count – (Must be completed within past 3 months)
- Day 6 – 10 Hysterosalpingogram – (Must be completed within past 12 months)
- Day 9 - 13 3D Ultrasound – (Must be completed within past 3 months)
- Day 6 – 10 Diagnostic Hysteroscopy – (Completed within past 6 months)
Male Requirements:
- Endocrine Panel: Testosterone, TSH, Prolactin, LH and FSH
- Direct sperm antibody test with semen analysis & morphology *
- DNA fragmentation with Semen analysis *
- *Need to be performed 2 – 3 weeks apart
Once the initial tests are done, they will be reviewed by CARE Fertility’s physician panel. The panel will determine the patient’s Tier based on the test results. There are three possible Tier placements for patients: Tier I (High Prognosis), II (Intermediate Prognosis), III (Low Prognosis). Tier pricing is as follows:

When the patient receives the Tier placement, he/she may then complete an agreement to begin their 12 months of treatment. The MaxCare© program includes all of the following types of treatment:
- In vitro fertilization (IVF) including associated patient anesthesia and embryo cryopreservation
- Frozen embryo transfer (FET)
- Intracytoplasmic sperm injection (ICSI)
- Intrauterine insemination (IUI)
- Ovulation induction and any service included in a routine IVF, FET, ICSI, IUI and Ovulation induction treatment cycle.
Several services are not included in the MaxCare© program. They are:
- Diagnostic testing
- Surgical procedures unrelated to the treatment cycle (i.e. diagnostic hysteroscopy, polypectomy, ovarian cystectomy, myomectomy)
- Complications of treatment (i.e. treatment for miscarriage, ectopic pregnancy, ovarian hyperstimulation syndrome)
- Obstetrical management
- Egg donation treatment cycle
- Gestational carrier treatment cycle
- Preimplantation genetic diagnosis (PGD)
- Procedures to extract sperm from testicles (and similar procedures)
- Laparoscopic egg retrieval
- Tubal embryo transfer
- Fees for donor sperm
- Medications
- Storage of cryopreserved embryos beyond the expiration of the MaxCare© agreement
- Annual embryo or sperm storage from treatment cycles prior to the MaxCare© agreement
- FET cycles using embryos cryopreserved prior to the effective date of the MaxCare© agreement
- Infectious disease screening
- Prenatal and genetic screening
- Any other service that is not part of a routine IVF, FET, IUI or ovulation induction treatment cycle
